Agreement entered into by and between COLUMBIA FINANCIAL INTERNATIONAL, INC., hereinafter referred to as the Company, and , the undersigned, hereinafter referred to as the Listee.  

 The Company and the Listee mutually agree as follows:

       1.  Member Law Firms shall be carried in the following online Directories and Services:  

Member Law Firms shall also have complimentary access to our Consulting Department which gives advice on Attorney marketing and Office Management., access to www.cfidatasearch.com (skip tracing), and  www.claimforwarders.com (electronic claims forwarding software).

        2.       Listees’ name shall be carried under www.columbialist.com under city/county of in the State of in the above listed online directories for one year commencing with the first day of .

        3.       In consideration whereof, the Listee agrees to pay the Company the sum of which shall be due and payable as follows: annually by check or credit card                                                       

        4.       The Company reserves the right for any reason deemed by it sufficient in the protection of its interests, at any point to terminate this agreement by refunding to the Listee the unearned amount of any sum paid, covering the then un-expired portion of this agreement.

        5.      The term for this agreement should be for a period of one year from above date and shall continue in force and in effect under the same terms and conditions as set forth herein from year to year (if applicable the Company will use the credit card on file for the purpose of annual renewal), unless sooner terminated by the Company as foresaid, or by the Listee by a written notice sent by registered mail to the Company not less than 60 days prior to the expiration of any contract.

        6.       This agreement shall not be effective until executed by the Company, by one of its’ duly authorized officers. It is with the understanding that as a Listee I am considered a bonded attorney in order to receive referrals.  It is also my understanding that this is for marketing purposes and the listing is not based upon either the quality or the amount of referrals that are sent to our office.

       7.        It is solely the responsibility of the Listee to inform the Company of any changes in their listing.
